Athletic Participation Packet

The online packet includes all the forms & information needed by
a student-athlete to participate in Alameda High School Athletics.

PACKET UPDATED 9/9/2011:

  • The forms are no longer available on the Booster website. You must now go to the AHS website to access/print these forms.

  • The paperwork has been updated and simplified.

  • Completed paperwork must be turned in to the Athletic Director; not to the coaches. (See details below.)

  • ALL Hornet athletes -- for Winter, Fall and Spring -- are to complete and turn in their paperwork at the same time, i.e., between Aug 15-17 & Aug 22-24.

Your participation packet must be completed, turned in to the Athletic Director, 
and then picked up at the office by your coach BEFORE you can tryout.
NO EXCEPTIONS !!!

COMPLETING/TURNING IN COMPLETED PAPERWORK:

  1. Go to the AHS website.

  2. Print out and complete all designated "Eligibility" paperwork.

  3. Turn in all completed paperwork to:

  • The Athletic Director's Office (by entrance to new gym).

-- OR -- 

  • In Media Center pit at lunch time on Tue, Wed, Thu only.

Please do NOT slide packet under office door or turn it into the coaches. 
(Check back at the AHS website for future updates.).

  ClarificationYour coach must have your medical release form in his/her hand BEFORE you can practice/try-out, so if you drop off your packet to the Athletics Director's office the same day as practice, you may have to wait a day before you can actually participate. Therefore, it's a good idea to get your packet submitted as early as possible.

and they can arrange for you to pick up a pre-printed copy of this packet.
